General Privacy Statement
BHC is committed to protecting the privacy and confidentiality of health plan
members’ identifiable health information in accordance with all applicable laws
including the Health Insurance Portability and Accountability Act (HIPAA).

Employees may only
use and disclose members’ protected health information in accordance with BHC
policies and practices.

Protected health
information may only be used or disclosed for proper use (i.e. payment,
treatment, quality issues, etc.) and only the minimum information needed to
accomplish the intended purpose may be disclosed. |

All other use of
protected health information requires the expressed written approval of the
member. |

Reasonable
precautions must always be taken to prevent inadvertent or unnecessary
disclosure of information. |

BHC members’
privacy rights are set forth in our Privacy Notice and describe the manner in
which medical information will be used and disclosed. The Privacy Notice also
contains the procedures on how members may request and obtain access to their
protected health information. |
